What is Approval and decision record?

An approval and decision record is a durable entry showing who approved, rejected or conditioned an AI-related decision, when, on what information and with what reasoning. It makes governance decisions traceable, so they can be reviewed by auditors, regulators and later owners.

Typical decisions include approving a system for use, accepting a residual risk, granting an exception or retiring a system. Strong records link to the system, the assessment and the evidence reviewed, and cannot be quietly edited afterwards. Approval routing often varies with risk level and role.
