Home/Glossary/AI incident

What is AI incident?

An AI incident is an event in which the development, deployment or use of an AI system causes, or nearly causes, harm or a breach of obligations, such as a harmful output, a discriminatory decision, a data leak or a safety failure. Organisations log incidents, investigate causes and track corrective actions.

Not every incident meets a legal reporting threshold. The EU AI Act defines a narrower category, the serious incident, which providers of high-risk systems must report to authorities. A sensible incident process records all events and escalates the serious ones.
