The Decision Ledger is the durable, accountable history of governance outcomes across their full lifecycle.

What a decision records

  • The governed resource, requested outcome, final status, rationale, and accountable approvers.
  • The evidence and risk context reviewed at the time of decision.
  • Conditions, effective period, expiry, revocation, delegation, and reassessment history where available.

Why the ledger matters

A queue shows work waiting for action; the ledger explains what authority existed, who granted it, on what basis, and whether it remains valid. Preserve prior decisions rather than overwriting history after change.

Global and Project views

Use the global view for cross-project decision history and work. Use the Project view when reviewing the local context of a specific engagement and its AI Systems.
