Governance does not end at approval. Monitoring determines whether the facts, controls, evidence, and assumptions behind a decision remain valid.

Monitoring lifecycle

Approved → Monitor → Material Change → Reopen affected assurance → Reassess → New Decision.

  • Create monitoring plans and scheduled checks.
  • Review runtime signals, thresholds, and breaches where configured.
  • Record material changes and analyze impact on the AI System and its governance state.

What can trigger reassessment

  • Changed purpose, users, autonomy, data, deployment, or jurisdiction.
  • Control failure, monitoring breach, incident, or significant performance drift.
  • Material vendor or model change.
  • Regulatory update or approval expiry.
